Acknowledgment of Transportation Services

I understand that I am voluntarily using transportation services provided by the above-named individual, driver, company, or transportation provider for travel from the designated pick-up location to the designated destination.

I acknowledge that travel by motor vehicle involves ordinary and reasonably foreseeable risks, including traffic delays, road conditions, accidents, sudden stops, vehicle-related incidents, and other circumstances that may occur during transportation.

Assumption of Risk and Release

To the fullest extent permitted by law, I voluntarily assume the ordinary risks associated with the transportation service and agree not to hold the transportation provider, driver, company, employees, agents, contractors, or representatives responsible for claims, damages, losses, or expenses arising from ordinary risks associated with transportation.

This waiver does not release any person or company from liability that cannot legally be waived, including liability arising from gross negligence, reckless misconduct, intentional wrongdoing, or other conduct for which liability may not lawfully be excluded.

Personal Property

I understand that I am responsible for securing and maintaining possession of my personal belongings before, during, and after transportation.

The transportation provider is not responsible for personal property that is lost, forgotten, stolen, misplaced, or damaged during transportation, including but not limited to:

* Cell phones and electronic devices
* Wallets, purses, bags, or luggage
* Cash, credit cards, or identification
* Jewelry or other valuables
* Clothing or personal items
* Medical equipment or assistive devices
* Any property brought into or stored in the vehicle

I understand that it is my responsibility to inspect the vehicle and retrieve all personal belongings upon arrival at my destination.

Passenger Acknowledgment

By checking this box, I confirm that I have read and understand this Transportation Service Liability and Property Waiver. I voluntarily accept the transportation services described above and acknowledge the terms regarding transportation risks and responsibility for personal property.
